For PPO plans, please provide your name, phone number, insurance name, member number, and date of birth in the email so that we can verify your insurance. 

You can also call or text us at 619-489-3130 during office hours Mon - Thurs 8am to 5pm and Friday 9am to 1pm.

The "Contact Us" box is the least effective way of scheduling an appt.
